Safe Space ACP Presentation

You never know when an unexpected event or illness might occur. Preparing your Advance Care Plan ensures the right decisions can be made in the event you cannot speak for yourself.

“Your Choice for Safe and Inclusive Healthcare” was created for members of the 2SLGBTQIA+ community by Canadian Virtual Hospice. This workshop is an opportunity to learn about the importance of having an Advanced Care Plan and completing your Health Care Directive. It can help you to have conversations with your healthcare providers to plan for your future care.

The Advance Care Planning Workbook can help you through the advance care planning process. It contains questions to help you think about your values and beliefs and what you would want others to know about your choice of health care and treatment options. This workbook can also be used to complete a Health Care Directive.

Some people avoid thinking or making decisions about their future needs. By giving thought to your wishes and preferences now and writing them down you can be better prepared for changes in your health. We understand decisions about your care need to be based on an inclusive approach that respects you and your wishes.

Join us on Wednesday, April 3rd from 6:00-9:00pm at a safe space presentation where the Advanced Care Plan workbook will be explained, what is a Health Care Directive, how to complete yours, and identifying your Workbooks and Health Care Directive forms will be provided.
