Are You a Healthcare Provider?

If you are a healthcare provider, you can refer your patient for Hospice PEI Services or Grief Support Services. Your patient can also self-refer if they wish. Referrals can be made by contacting the Volunteer Coordinator in their region. To see our staff directory please click here.

Our philosophy of care for a patient with a life-limiting or palliative diagnosis is providing the best care and support that enhances and supports the healthcare team. As a member of the Integrated Palliative Care team, Hospice PEI has the resources to help the patient and their family on their journey.

Patients who have a life-limiting or palliative diagnosis can be referred for services. Some examples of life-limiting illnesses are cancer, ALS, advanced dementia, end-stage COPD, and end-stage heart, lung, and kidney disease. The patient does not need to be a part of the Integrated Palliative Care Program to receive our services.

Any patient is eligible for our grief support services, regardless of any prior involvement with Hospice PEI or palliative care. Anyone requesting grief support services from us must self refer.
