Are you living with a life-limiting or palliative diagnosis? Hospice PEI can help you and your loved ones on your journey.

Who Can Refer?
Anyone living with a life-limiting or palliative diagnosis can request Hospice PEI support services. Some examples of a life-limiting illness are cancer, ALS, advanced dementia, end-stage COPD, and end-stage heart, lung, and kidney disease.
How To Refer
Anyone can make a referral for support services. A person can self-refer or referrals can be made on behalf of someone by their loved ones or by healthcare professionals. To make a referral, please call the Volunteer Coordinator in your area. Click here to see our staff directory.
